# Cardiac troponin as a lab test

Cardiac troponin marks heart muscle injury; calling it a heart attack needs a rise or fall and a setting of ischaemia.

Updated 2026-11-01

## Key facts

- Cardiac troponins I and T are myocardial isoforms distinct from skeletal muscle ones: highly specific for myocardial injury.
- Myocardial injury = troponin above the 99th percentile of a reference population (the exact value depends on the assay).
- Infarction = acute injury (a rising or falling curve) + evidence of ischaemia (symptoms, ECG, imaging) by the universal definition.
- It rises at 2–4 hours (sooner with high-sensitivity assays), peaks at 24–48 h and may stay high for 7–14 days.
- High-sensitivity assays allow fast 0/1 h or 0/2 h rule-out and rule-in pathways.

## In detail

- What it measures: proteins of the troponin complex (C, I, T) that regulate actin–myosin binding by calcium; cardiac I or T is measured by immunoassay.
- Where it comes from: a small cytosolic pool leaks first, then the sarcomere-bound fraction, which is why the rise lasts days.
- Why it rises (without type 1 infarction): myocarditis, heart failure, pulmonary embolism, sepsis, tachyarrhythmias, aortic dissection, contusion, cardiotoxicity, chronic kidney disease, extreme exercise, stroke.
- Pitfalls: a single high value is not enough; what defines acuteness is the change between two samples. Chronic kidney disease raises baseline troponin steadily.
- Heterophile antibodies, rheumatoid factor or macrotroponin can give false positives; high-dose biotin interferes with some immunoassays.

## Clinical notes

- Type 2 infarction comes from an oxygen supply–demand mismatch (anaemia, hypotension, tachycardia), without plaque rupture.
- A normal troponin very early does not rule out infarction if pain began recently: it is repeated by the laboratory's pathway.
- In pulmonary embolism a high troponin marks a worse prognosis from right ventricular strain.

## Test yourself

Think of the answer, then tap the question to see it.

## What defines myocardial injury biochemically?

A cardiac troponin above the 99th percentile of the reference population for the assay used.

## What is needed besides a high troponin to diagnose infarction?

A rise or fall of troponin (acute injury) plus evidence of ischaemia: symptoms, ECG changes or imaging abnormalities.

## Why does troponin stay high for several days?

After the cytosolic pool, sarcomere-bound troponin is released slowly as it breaks down.

## Which causes other than type 1 infarction raise troponin?

Myocarditis, pulmonary embolism, sepsis, heart failure or chronic kidney disease, among others.
