# Bleeding syndromes: platelet vs coagulation pattern

- Syndrome

Abnormal bleeding: the kind of skin lesion and the site of bleeding show whether platelets or clotting factors are failing.

## Key points

- Platelet pattern: petechiae, purpura, mucosal bleeding (gums, nose, heavy periods) and immediate bleeding after a cut.
- Coagulation pattern: hemarthroses, deep muscle hematomas and delayed bleeding after surgery or a dental extraction.
- Petechiae do not blanch under pressure (diascopy), unlike erythema.
- Palpable purpura suggests vasculitis, not thrombocytopenia.
- Hemophilia A (factor VIII) and B (factor IX) are X-linked: they affect almost only males.
- In Colombia, fever with petechiae and a positive tourniquet test should bring dengue to mind.

## What it is

Bleeding syndromes are disorders in which blood does not stop as it should, because of a failure of primary hemostasis (platelets and vessels) or secondary hemostasis (clotting factors). At the bedside, the type of lesion, the site of bleeding and its timing after an injury separate the two patterns before any laboratory test.

## Key findings

- Skin and mucosa: petechiae, purpura, ecchymoses, blood blisters in the mouth.
- Joints and muscles: hemarthrosis (knee, ankle, elbow) and deep hematomas.
- Oozing from puncture and catheter sites (disseminated intravascular coagulation).
- Liver and spleen: liver disease or splenomegaly point to a cause other than immune thrombocytopenia.
- Family history and sex of the patient; response to past surgery and extractions.
- Drugs: antiplatelets, NSAIDs, anticoagulants and heparin.

## Pathophysiology

- Primary hemostasis: platelets stick to exposed collagen with the help of von Willebrand factor and form a plug that seals small capillary injuries; when it fails, skin and mucosa bleed at the moment of injury.
- Secondary hemostasis: the clotting cascade generates thrombin, which turns fibrinogen into fibrin and reinforces the plug; when it fails, the first plug forms but breaks down, and bleeding returns hours later in deep tissues.
- The liver makes most clotting factors, and several need vitamin K: liver disease and vitamin K deficiency give a coagulation pattern.
- In disseminated intravascular coagulation platelets and factors are consumed together: both patterns mix, with oozing from puncture sites.

## Platelet pattern vs coagulation pattern

| Feature | Platelet (primary hemostasis) | Coagulation (secondary hemostasis) |
| --- | --- | --- |
| Typical skin lesion | Petechiae and purpura | Large ecchymoses and hematomas |
| Site | Skin and mucosa: gums, nose, periods | Deep tissues: joints and muscles |
| Timing after an injury | Immediate | Delayed, hours later |
| Hemarthrosis | Rare | Typical |
| Inheritance and sex | More in women; von Willebrand, autosomal | X-linked hemophilias: males |
| Examples | Immune thrombocytopenia, dengue, antiplatelets | Hemophilia, vitamin K deficiency, liver disease |
| Screening tests | Platelet count and platelet function | Prothrombin time and partial thromboplastin time |

## Common causes

- Platelet: immune thrombocytopenia, dengue, leukemias and marrow aplasia, thrombotic thrombocytopenic purpura, heparin-induced thrombocytopenia, antiplatelet drugs and von Willebrand disease.
- Coagulation: hemophilia A and B, vitamin K deficiency (including in the newborn), liver disease and anticoagulants.
- Mixed: disseminated intravascular coagulation in sepsis, trauma, obstetric complications and cancer.
- Vascular: vasculitis with palpable purpura, scurvy and skin fragility from age or corticosteroids.

## Tests that confirm it

- Blood count with platelet count and a peripheral smear.
- Prothrombin time (extrinsic pathway) and activated partial thromboplastin time (intrinsic pathway).
- Fibrinogen and D-dimer if disseminated intravascular coagulation is suspected; factor assays and von Willebrand studies as needed.

## Memory aid

'Platelets paint the skin; factors fill the knee': red dots for platelets, blood in the joint for clotting.

## Test yourself

Think of the answer, then tap the question to see it.

## Which bleeding pattern suggests platelet failure?

Petechiae, purpura and mucosal bleeding (gums, nose, heavy periods) that appears immediately after an injury.

## Which finding is typical of hemophilia and rare in thrombocytopenia?

Hemarthrosis, bleeding into the joints, along with deep muscle hematomas and delayed bleeding after surgery.

## How is a petechia told apart from a spot of erythema on examination?

Pressing with a glass slide or finger (diascopy): a petechia does not fade because the blood is outside the vessel; erythema blanches.

## Why does liver disease cause a coagulation pattern?

The liver makes most clotting factors, including the vitamin K-dependent ones; when it is diseased, the prothrombin time lengthens.
