## What it is

Very common benign changes of the breast tissue, with lumpiness, cysts and pain that typically vary with the menstrual cycle. They are not a disease but a response of breast tissue to hormones.

## Who gets it

The most frequent benign breast condition, mostly in women between their 20s and menopause. It is one of the most common reasons for breast consultations, and symptoms usually ease after menopause.

## Causes

- Hormonal response of breast tissue to cyclical oestrogen and progesterone
- Relative oestrogen excess
- Hormone therapy in some postmenopausal women

## Risk factors

- Reproductive age
- Hormone replacement therapy
- Nulliparity and late first pregnancy (variable evidence)
- Family history of benign breast disease

## Pathophysiology

Repeated cycles of hormonal stimulation make the stroma become fibrotic, ducts and lobules proliferate and small dilated ducts form cysts. Most changes are non-proliferative and carry no extra cancer risk; proliferative lesions, especially with atypia, raise risk.

## Classification

- Non-proliferative (cysts, fibrosis, mild hyperplasia): no increased risk
- Proliferative without atypia (for example usual ductal hyperplasia, sclerosing adenosis): slightly increased risk
- Atypical hyperplasia: clearly increased risk

## Symptoms and signs

- Cyclical breast pain and tenderness, worse before menstruation
- Diffuse lumpiness or nodularity, often in the upper outer quadrants
- Bilateral symptoms
- Cysts that change in size over the cycle
- Occasional clear or green-yellow [nipple](https://durgical.com/en/anatomy/nipple/) discharge

## Red flags

- A dominant lump that persists after the period
- Bloody or single-duct spontaneous nipple discharge
- [Skin](https://durgical.com/en/anatomy/skin/) dimpling or nipple retraction
- Axillary lymph nodes
- Cyst fluid with blood or a residual mass after aspiration

## Diagnosis

**Clinical**
History of cyclical pain and nodularity; examination, ideally after the period, looks for a dominant mass.

**Tests**
Fine-needle aspiration of cysts; core biopsy of solid or suspicious lesions.

**Imaging**
Ultrasound in younger women and to tell cysts from solid masses; mammography by age and risk.

## Differential diagnosis

- Fibroadenoma
- Breast cancer
- Mastitis
- Non-cyclical breast pain
- [Chest](https://durgical.com/en/anatomy/region/chest/) wall pain (costochondritis)

## Goal of treatment

Relieve symptoms, reassure once cancer is excluded and identify the few lesions that increase cancer risk.

## Treatment

**Conservative**
Reassurance, a well-fitting supportive bra and pain diary; reducing caffeine is often suggested, though evidence is limited.

**Medical**
Oral or topical non-steroidal anti-inflammatory drugs; hormonal agents are kept for severe refractory pain because of side effects.

**Procedure**
Aspiration of large or painful cysts; biopsy or excision of a persistent or suspicious mass.

**Follow-up**
Follow-up and closer surveillance when biopsy shows atypia.

## Complications

- Anxiety and repeated testing
- Difficulty detecting a new lump in nodular breasts
- Higher cancer risk only with atypical hyperplasia

## Prognosis

Benign. Symptoms fluctuate and usually settle after menopause; long-term outlook depends only on the lesion type found on biopsy.

## Test yourself

Think of the answer, then tap the question to see it.

## What is the typical pattern of pain in fibrocystic breast changes?

Cyclical, bilateral pain and tenderness that worsens before menstruation and eases afterwards.

## Which fibrocystic lesion clearly increases breast cancer risk?

Atypical hyperplasia (ductal or lobular); non-proliferative changes do not increase risk.

## Which imaging test is preferred first in a young woman with a breast lump?

Ultrasound, which also distinguishes a cyst from a solid mass.

## When does a breast cyst need further study after aspiration?

When the fluid is bloody, a mass remains after aspiration or the cyst recurs quickly.
