# The neutropenic and immunosuppressed patient (nursing)

- Topic

With very few neutrophils, a fever may be the only sign of a dangerous infection, so prevention and fast action matter.

Updated 2026-11-01

## Key points

- ANC = total white cells × (percent segmented neutrophils + percent bands); severe neutropenia is an ANC below 500 cells per microliter.
- Hand hygiene is the single most important measure, for staff, visitors and the patient.
- Fever in a neutropenic patient is an emergency: blood cultures and broad-spectrum IV antibiotics within the first hour, without waiting for results.
- Avoid rectal temperatures, suppositories and enemas (risk of mucosal injury and bacteremia).
- Keep away fresh flowers, potted plants and standing water; limit visitors who are sick or recently received a live vaccine; the patient should not clean litter boxes or bird cages.
- Prefer well-cooked food and washed or peeled produce; avoid raw meat, eggs, fish and unpasteurized products.

## What it is

Neutropenia is a low absolute neutrophil count (ANC), most often from chemotherapy. Because neutrophils make pus and redness, infection signs are muted and fever may be the only clue; febrile neutropenia is an oncologic emergency. Nursing care combines protective measures to prevent infection with close watching and a rapid response to fever.

## Key facts

- Antipyretics are not given routinely before the patient is assessed, because they can hide the fever that signals infection.
- The usual germs are the patient's own: gram-negative rods from the gut (including Pseudomonas), gram-positive cocci from the skin and catheters, and Candida; prolonged neutropenia adds Aspergillus.
- Allogeneic stem cell transplant recipients are often placed in a protective environment: positive-pressure room with HEPA-filtered air.
- Live vaccines are contraindicated in severely immunosuppressed patients.

## Step by step

- Assess: check the ANC, vital signs and temperature regularly, and inspect the mouth, skin, perianal area and catheter sites every shift.
- Protect: private room, strict hand hygiene, dedicated equipment, no plants or flowers, mask for the patient when leaving the room.
- Recognize: report fever, chills, new cough, sore throat, mouth sores, painful urination or redness at a catheter site at once.
- Act on fever: take blood cultures (peripheral and from each catheter lumen) and other cultures as indicated.
- Treat promptly: start ordered empiric broad-spectrum IV antibiotics within the first hour of fever.
- Reassess: monitor for signs of sepsis (hypotension, tachycardia, confusion, low urine output) and escalate early.
- Educate: teach the patient and family hand hygiene, food safety, mouth care and to report fever immediately.

## Clinical notes

- NCLEX priority: in a neutropenic patient, a new fever is acted on before routine tasks; the nurse does not assign this patient alongside patients with active infections.
- Transplant recipients on immunosuppressants need the same vigilance, with added risk of CMV, Pneumocystis and fungal infection.

## Test yourself

Think of the answer, then tap the question to see it.

## How is the absolute neutrophil count calculated?

Total white blood cell count multiplied by the combined percentage of segmented neutrophils and bands.

## Why may a neutropenic patient with pneumonia show little on the chest exam or X-ray?

Neutrophils produce the pus and inflammation that create signs; without them, fever may be the only sign.

## A nurse finds a temperature of 38.5 °C in a patient with an ANC below 500 cells per microliter. What is the priority?

Notify the provider, obtain blood cultures and start the ordered IV antibiotics as soon as possible, ideally within the first hour.

## Which ways of taking a temperature or giving medicine are avoided in neutropenia?

Rectal thermometers, suppositories and enemas, because they can injure the mucosa and allow bacteria into the blood.
