# Dipylidium caninum

- Parasite
- For completeness

A dog and cat tapeworm that passes to toddlers when they swallow a flea. Segments like rice grains show up in the nappy.

Updated 2026-11-01

Also called: dog tapeworm, flea tapeworm, dipylidiasis

## Key points

- Toddler + pet with fleas + rice grains in the nappy = Dipylidium.
- Intermediate host: the flea (Ctenocephalides); infection comes from swallowing it, not from eating meat.
- Proglottid with two genital pores and egg packets.
- Treatment with praziquantel and flea control for the pet.

## At a glance

**Family**
Dipylidiidae

**Type**
Cestode (tapeworm)

**Size**
Adult tens of centimetres long; gravid proglottids about a centimetre

## Morphology

- Proglottids with two genital pores, one on each side: the feature that gives it its name.
- Fresh segments are motile and shaped like pumpkin or cucumber seeds; dried, they look like rice grains.
- Eggs grouped in packets (egg capsules) of several eggs, each with a six-hooked embryo.

## Transmission

- Accidental swallowing of a dog or cat flea (Ctenocephalides) carrying the cysticercoid larva.
- Most frequent in toddlers who play with flea-infested pets; not spread from person to person.

**Reservoir:** Dogs and cats

### Isolation precautions

What the care team adds to standard precautions.

- Standard

## Life cycle

- The dog or cat sheds gravid proglottids in faeces or through the anus.
- Flea larvae eat the eggs in the environment.
- The cysticercoid larva develops inside the flea as the flea matures.
- The pet (or a child) swallows the infected adult flea.
- In the small intestine the larva attaches and grows into an adult tapeworm within weeks.

## Virulence factors

- It attaches to the mucosa with a hooked scolex and causes little damage.
- Segments leaving through the anus cause perianal itch.

## Diseases it causes

- Dipylidiasis: an almost always mild intestinal infection in children

## Clinical picture

- Usually asymptomatic; parents see motile segments in the nappy or clothes, sometimes mistaken for maggots.
- Anal itch, mild digestive upset.

## Diagnosis

- Identifying proglottids with two genital pores or egg packets in stool or the nappy.
- Flotation can miss them because the egg packets are heavy.

## Treatment

Anthelmintic against cestodes: praziquantel. The pet must also be treated and its fleas controlled to prevent reinfection.

Drug class only; doses and regimens are set by the clinician from the current guideline.

In Pharmacology:

- [Praziquantel](https://durgical.com/en/pharmacology/praziquantel/)

## Prevention

- Flea control and regular deworming of dogs and cats.
- Children's handwashing after playing with animals.

## In Colombia

It occurs in Colombia as worldwide, linked to pets with fleas; it mainly affects children under five.

## Memory aid

Di-pylidium, two doors: each segment has two genital pores, and the way in is a swallowed flea.

## Test yourself

Think of the answer, then tap the question to see it.

## How does a child become infected with Dipylidium caninum?

By accidentally swallowing a dog or cat flea that carries the cysticercoid larva.

## Which proglottid feature identifies Dipylidium?

It has two genital pores, one on each side of the segment.

## What do parents see in dipylidiasis?

Whitish segments moving in the nappy or stool that, once dry, look like rice grains.

## What the colours mean

**Gram-positive**
Thick peptidoglycan wall: stains purple.

**Gram-negative**
Thin wall with an outer membrane: stains pink.

**Acid-fast**
Lipid-rich wall (mycobacteria): Ziehl–Neelsen stain.
