# Edoxaban

A once-daily oral factor Xa inhibitor for atrial fibrillation and venous thrombosis. Half of it leaves through the kidneys, so kidney function guides its use.

Updated 2026-11-01

- Class: [Direct oral anticoagulants (DOACs)](https://durgical.com/en/pharmacology/classes/direct-oral-anticoagulants/)
- Enzyme: Coagulation factor Xa
- ATC code: B01AF03

## ⚠ High-alert medication

An error with this medicine can cause serious harm: double checks and protocols are used.

## Mechanism of action

Edoxaban binds factor Xa directly, without antithrombin, and blocks both free and clot-associated factor Xa. Less factor Xa means less thrombin, so fibrin formation and thrombin-driven platelet aggregation are both suppressed.

- Edoxaban binds the active site of factor Xa
- Free and clot-associated Xa are blocked
- Less thrombin forms, so less fibrin and less platelet activation

## Pharmacokinetics

**Absorption**
Oral bioavailability about 62%; food does not change it. Peak levels 1 to 2 hours after a dose.

**Distribution**
About 55% bound to plasma proteins, so it is not removed by dialysis in a useful way.

**Metabolism**
Minimal; only a small part goes through CYP3A4. It is a P-glycoprotein substrate.

**Elimination**
About half of the absorbed drug leaves unchanged in the urine.

## Indications

- Prevention of stroke and systemic embolism in nonvalvular atrial fibrillation. [Atrial fibrillation →](https://durgical.com/en/conditions/atrial-fibrillation/)
- Treatment of deep vein thrombosis, started after at least 5 days of heparin, low-molecular-weight heparin or fondaparinux. [Deep vein thrombosis →](https://durgical.com/en/conditions/deep-vein-thrombosis/)
- Treatment of pulmonary embolism, with the same initial parenteral course. [Pulmonary embolism →](https://durgical.com/en/conditions/pulmonary-embolism/)

## Adverse effects

### Common

- Bleeding: gums, nose, heavy menstrual bleeding, urine
- More gastrointestinal bleeding than with warfarin

### Serious (rare, important)

- Serious, sometimes fatal bleeding
- Spinal or epidural haematoma with neuraxial anaesthesia or spinal puncture
- Stroke if it is stopped early in atrial fibrillation

## Contraindications

- Active pathological bleeding
- End-stage kidney failure
- Not recommended: mechanical heart valves, antiphospholipid syndrome; in the United States, also very high kidney function in atrial fibrillation

## Key interactions

**Potent P-glycoprotein inhibitors (quinidine, dronedarone, erythromycin, ketoconazole, cyclosporine)**
Raise edoxaban exposure; the regimen is reduced.

**Rifampicin**
Lowers edoxaban levels; combined use is avoided.

**Antiplatelets, NSAIDs, other anticoagulants**
Raise the risk of bleeding.

## What is monitored

- Kidney function before starting and at least yearly
- Signs of bleeding and haemoglobin when needed
- Routine PT or INR is not useful; drug-calibrated anti-Xa assays exist

## Administration and safety

- Oral, once a day, with or without food
- For people who cannot swallow, tablets can be crushed and mixed with water or applesauce, or given through a gastric tube
- In venous thrombosis it is started only after the initial parenteral anticoagulant course
- For life-threatening bleeding, four-factor prothrombin complex concentrate may be considered; andexanet is approved for apixaban and rivaroxaban

## Nursing care

- Check kidney function and body weight before the first dose
- Watch for bleeding: black stools, blood in urine, heavy periods, vomiting blood
- After spinal or epidural procedures, check for back pain, numbness or weakness
- Teach the patient not to stop it on their own and to tell every dentist and surgeon

## Pregnancy and breastfeeding

Data in pregnancy are limited; pregnancy is discussed with the treating team before and during use.

## Memory aid

EDOXaban: "-xaban" blocks Xa, once a day, and the kidneys hold half of the key.

## Test yourself

Think of the answer, then tap the question to see it.

## How is edoxaban started for deep vein thrombosis, compared with rivaroxaban and apixaban?

Edoxaban starts only after at least 5 days of heparin, low-molecular-weight heparin or fondaparinux; rivaroxaban and apixaban can be started straight away.

## Which type of bleeding is more common with edoxaban than with warfarin?

Gastrointestinal bleeding; intracranial bleeding, in contrast, is less common.

## Why can dialysis not remove edoxaban in a useful way?

Like the other oral factor Xa inhibitors, it is too protein-bound for dialysis to clear it effectively.
