# Flumazenil

A competitive antagonist at the benzodiazepine site of the GABA-A receptor. It reverses benzodiazepine sedation but can trigger seizures.

- Receptor: GABA-A receptor, benzodiazepine site (α1/γ2 interface)
- ATC code: V03AB25

◎**GABA-A receptor, benzodiazepine site (α1/γ2 interface)**Flumazenil

ⓘ The figure shows the human α1β2γ2 GABA-A receptor bound to GABA and flumazenil, solved by cryo-electron microscopy.

**Target in 3D:** GABA-A receptor, benzodiazepine site (α1/γ2 interface). PDB structure 6X3U (RCSB PDB, public domain). [View the structure at RCSB PDB](https://www.rcsb.org/structure/6X3U) · Drag to rotate · pinch to zoom

## Mechanism of action

Benzodiazepines bind between the α and γ subunits of the GABA-A receptor and make GABA open the chloride channel more often. Flumazenil occupies that same site without activating it, so it displaces the benzodiazepine and reverses sedation and respiratory depression. It does not reverse ethanol, barbiturates or general anaesthetics, which act at other sites. In a dependent patient it can provoke abrupt withdrawal with seizures.

- Benzodiazepines boost GABA at the α/γ site
- Flumazenil competes for that same site
- Sedation and respiratory depression are reversed
- Short action: sedation may return

## Pharmacokinetics

**Distribution**
Distributes quickly outside the blood; about half bound to plasma proteins

**Metabolism**
Liver; clearance falls sharply in liver disease

**Half-life**
About an hour, shorter than most benzodiazepines

**Onset and duration**
Within minutes; each dose takes about 6 to 10 minutes to reach its full effect

## Indications

- Reversal of benzodiazepine sedation after procedures or anaesthesia (iatrogenic over-sedation)
- Selected cases of benzodiazepine overdose; it is not used routinely in unknown overdoses in the emergency department

## Adverse effects

### Common

- Agitation, anxiety and emotional lability
- Nausea and vomiting
- Dizziness
- Pain at the injection site

### Serious (rare, important)

- Seizures, especially in benzodiazepine dependence or tricyclic antidepressant co-ingestion
- Arrhythmias
- Resedation once flumazenil wears off
- Raised intracranial pressure in head injury

## Contraindications

- A benzodiazepine given to control a life-threatening condition (status epilepticus, raised intracranial pressure)
- Signs of serious tricyclic antidepressant overdose
- Hypersensitivity to flumazenil or benzodiazepines

## Key interactions

**Tricyclic antidepressants and other proconvulsant drugs**
Removing the benzodiazepine protection unmasks seizures and arrhythmias

**Long-acting or large doses of benzodiazepines**
They outlast flumazenil, so sedation can return

## What is monitored

- Level of consciousness and respiratory rate for a prolonged period after the last dose
- Seizure activity
- ECG, especially if a tricyclic may be involved

## Administration and safety

- Given intravenously in small increments, titrated to effect
- Through a freely running infusion in a large vein, to limit pain and irritation
- Airway and ventilation are secured first; it does not replace ventilatory support

## Nursing care

- Ask about long-term benzodiazepine use and seizure history before it is given
- Have seizure treatment ready
- Observe for resedation and do not discharge early

## Pregnancy and breastfeeding

There are no adequate studies in pregnancy; it is used only when the benefit clearly outweighs the risk.

## Memory aid

Flumazenil flips the benzo switch off, briefly: short action means resedation, and a dependent brain may answer with seizures.

Think of the answer, then tap the question to see it.

## Where does flumazenil bind?

To the benzodiazepine site of the GABA-A receptor, at the α/γ subunit interface, as a competitive antagonist.

## Why is flumazenil not used routinely in unknown overdoses?

It can cause seizures in benzodiazepine dependence or when tricyclics or other proconvulsants were also taken.

## Why can a patient become sedated again after flumazenil?

Its half-life is about an hour, shorter than that of many benzodiazepines.

## Does flumazenil reverse alcohol or barbiturate sedation?

No; those act on other sites of the GABA-A receptor.
