◎**Insulin receptor**Insulin aspart

ⓘ The figure shows human insulin bound to site 1 of an insulin receptor fragment. Aspart differs only at B28 and binds the same site.

## Mechanism of action

In insulin aspart the proline at B28 is replaced by aspartic acid. The added negative charge reduces self-association, so the hexamers dissociate rapidly into monomers after injection and are absorbed faster than regular insulin. Once in the blood it acts on the insulin receptor exactly like human insulin.

- Aspartate at B28 reduces self-association
- Monomers are absorbed rapidly from under the skin
- Activates the insulin receptor like human insulin
- Covers the glucose rise of a meal

## Pharmacokinetics

**Absorption**
Subcutaneous, faster than regular insulin; intravenous use only under medical supervision.

**Half-life**
Apparent half-life after a subcutaneous injection of about 81 minutes in the label's study.

**Onset and duration**
Injected 5 to 10 minutes before a meal; the greatest glucose-lowering effect occurs between 1 and 3 hours after injection.

## Indications

- Mealtime and correction insulin in type 1 diabetes, by injection or pump [Type 1 diabetes mellitus →](https://durgical.com/en/conditions/type-1-diabetes-mellitus/)
- Mealtime insulin in type 2 diabetes [Type 2 diabetes mellitus →](https://durgical.com/en/conditions/type-2-diabetes-mellitus/)

## Adverse effects

### Common

- Hypoglycemia
- Weight gain
- Injection-site reactions and lipodystrophy
- Low potassium

### Serious (rare, important)

- Severe hypoglycemia
- Hyperglycemia and ketoacidosis if an insulin pump fails
- Severe allergic reactions (rare)

## Contraindications

- During an episode of hypoglycemia
- Hypersensitivity to insulin aspart or an excipient

## Key interactions

**Beta-blockers, clonidine**
Can blunt the warning signs of hypoglycemia.

**Other glucose-lowering drugs**
Raise the risk of hypoglycemia.

**Corticosteroids, diuretics, atypical antipsychotics, niacin**
Reduce the glucose-lowering effect.

**Thiazolidinediones (pioglitazone)**
Fluid retention and possible heart failure.

## What is monitored

- Blood glucose before and after meals
- Glucose and potassium closely during intravenous use
- HbA1c
- Pump and injection sites

## Administration and safety

- Subcutaneous injection shortly before a meal, or continuous subcutaneous infusion by pump.
- Intravenous use only under medical supervision, with close checks of glucose and potassium.
- In a pump it is not diluted or mixed with other insulins.
- Look-alike risk with premixed aspart products: the label is checked before every dose.

## Nursing care

- Before: blood glucose and confirmation that the meal is available.
- After: watch for hypoglycemia in the first hours.
- Review the dose with the team if the patient cannot eat.
- Teach pump users to recognize pump failure and to have a backup injection plan.

## Pregnancy and breastfeeding

Rapid analogues such as aspart are used in pregnancy; insulin needs change by trimester and glucose is checked often.

## Memory aid

«Aspart = Aspartate at the start of the meal»: one aspartate at B28 and the insulin is ready in minutes.

## Test yourself

Think of the answer, then tap the question to see it.

## Which single change makes insulin aspart rapid acting?

Proline B28 is replaced by aspartic acid, which reduces self-association so the insulin dissociates quickly into monomers.

## When does aspart's glucose-lowering effect peak after a subcutaneous injection?

Between about 1 and 3 hours after the injection.

## What extra monitoring is needed when aspart is given intravenously?

Close blood glucose and potassium monitoring, under medical supervision, to avoid hypoglycemia and hypokalemia.
