# Pioglitazone

An oral insulin sensitizer (PPAR-gamma agonist). It lowers glucose slowly without hypoglycemia, but retains fluid and can worsen heart failure.

- Class: [Thiazolidinediones (glitazones)](https://durgical.com/en/pharmacology/classes/thiazolidinediones/)
- Receptor: PPAR-gamma (nuclear receptor)
- ATC code: A10BG03

◎**PPAR-gamma (nuclear receptor)**Pioglitazone

ⓘ The figure shows pioglitazone in the ligand-binding domain of human PPAR-gamma.

**Target in 3D:** PPAR-gamma (nuclear receptor). PDB structure 5Y2O (RCSB PDB, public domain). [View the structure at RCSB PDB](https://www.rcsb.org/structure/5Y2O) · Drag to rotate · pinch to zoom

## Mechanism of action

Pioglitazone activates PPAR-gamma, which pairs with RXR on DNA and switches on genes for fat storage, glucose uptake (GLUT4) and adiponectin. Fat is moved into subcutaneous fat cells and away from liver and muscle, so these tissues respond better to insulin; hepatic glucose output falls. It also lowers triglycerides and raises HDL cholesterol. The renal collecting duct retains sodium under PPAR-gamma activation, causing edema.

- Enters the cell and binds PPAR-gamma in the nucleus
- PPAR-gamma and RXR switch on fat-storage and insulin-action genes
- Fat leaves liver and muscle; insulin sensitivity improves
- Glucose falls over weeks; the kidney retains salt and water

## Pharmacokinetics

**Absorption**
Oral, well absorbed.

**Distribution**
Highly bound to plasma proteins.

**Metabolism**
In the liver, mainly by CYP2C8 (and CYP3A4), to active metabolites.

**Onset and duration**
Full effect on glucose takes weeks to a few months.

## Indications

- Type 2 diabetes, with diet and exercise, alone or combined [Type 2 diabetes mellitus →](https://durgical.com/en/conditions/type-2-diabetes-mellitus/)

## Adverse effects

### Common

- Edema and weight gain
- Upper respiratory infections, headache, muscle pain
- Mild fall in hemoglobin and hematocrit

### Serious (rare, important)

- Heart failure, new or worse
- Fractures, especially in women
- Possible increased risk of bladder cancer
- Macular edema
- Liver failure (rare reports)

## Contraindications

- Starting in established NYHA class III or IV heart failure
- Hypersensitivity to pioglitazone
- Active bladder cancer (not used)

## Key interactions

**Gemfibrozil and other strong CYP2C8 inhibitors**
Raise pioglitazone levels.

**Rifampicin (CYP2C8 inducer)**
Lowers pioglitazone levels.

**Insulin**
More fluid retention and heart failure; also more hypoglycemia.

**Topiramate**
Can lower pioglitazone levels.

## What is monitored

- Weight, edema and signs of heart failure
- Liver tests before starting and if symptoms appear
- HbA1c
- Blood in urine (bladder cancer)

## Administration and safety

- Oral tablet, once a day, with or without food.
- Several weeks are needed before judging the effect.

## Nursing care

- Before: heart failure history, liver tests, bladder cancer history.
- During: daily weight, ankle swelling, breathlessness; report fast weight gain.
- Teach to report dark urine, jaundice, blood in urine or vision changes.
- Women who did not ovulate may become fertile again; contraception is discussed.

## Pregnancy and breastfeeding

Data in pregnancy are limited; insulin is the usual choice.

## Memory aid

Glitazones 'sponge up' fat into fat cells and also sponge up water: better insulin action, but puffy ankles and a heavier load for a weak heart.

## Test yourself

Think of the answer, then tap the question to see it.

## Why can pioglitazone worsen heart failure?

PPAR-gamma activation makes the kidney retain sodium and water, expanding plasma volume and causing edema.

## Which fibrate raises pioglitazone levels, and why?

Gemfibrozil, because it strongly inhibits CYP2C8, the main enzyme that breaks pioglitazone down.

## Does pioglitazone cause hypoglycemia when used alone?

No. It improves insulin sensitivity but does not increase insulin secretion; hypoglycemia appears mainly with insulin or sulfonylureas.

## Which cancer is a warning for pioglitazone?

Bladder cancer: it is not used in active bladder cancer and blood in the urine is reported promptly.
