# Pramlintide

An injected copy of amylin, a beta-cell hormone, used with mealtime insulin. It can cause severe hypoglycemia in the hours after injection.

- Receptor: Amylin receptors (calcitonin receptor with RAMP proteins)
- ATC code: A10BX05

◎**Amylin receptors (calcitonin receptor with RAMP proteins)**Pramlintide

ⓘ The figure shows a pramlintide analogue peptide (San45, not pramlintide itself) bound to the human amylin 3 receptor (calcitonin receptor plus RAMP3) with its Gs protein.

**Target in 3D:** Amylin receptors (calcitonin receptor with RAMP proteins). PDB structure 8F2B (RCSB PDB, public domain). [View the structure at RCSB PDB](https://www.rcsb.org/structure/8F2B) · Drag to rotate · pinch to zoom

## Mechanism of action

Amylin is co-secreted with insulin by beta cells and is missing in type 1 diabetes and reduced in advanced type 2. Pramlintide is amylin with amino-acid changes that stop it from clumping. Acting on amylin receptors in the hindbrain, it slows gastric emptying, lowers glucagon after meals and increases satiety, so glucose rises less after eating.

- Activates amylin receptors in the hindbrain
- The stomach empties more slowly; fullness comes sooner
- Glucagon after meals falls
- Smaller glucose rise after meals

## Pharmacokinetics

**Absorption**
Subcutaneous injection before major meals.

**Elimination**
Mainly by the kidney.

**Half-life**
Short; it acts around the meal it precedes.

## Indications

- Type 1 diabetes, with mealtime insulin [Type 1 diabetes mellitus →](https://durgical.com/en/conditions/type-1-diabetes-mellitus/)
- Type 2 diabetes in patients using mealtime insulin [Type 2 diabetes mellitus →](https://durgical.com/en/conditions/type-2-diabetes-mellitus/)

## Adverse effects

### Common

- Nausea (most common, mostly at the start)
- Reduced appetite, vomiting
- Injection-site reactions

### Serious (rare, important)

- Severe insulin-induced hypoglycemia, highest in the first hours after injection and in type 1 diabetes

## Contraindications

- Gastroparesis
- Hypoglycemia unawareness or repeated severe hypoglycemia
- Hypersensitivity to pramlintide

## Key interactions

**Insulin**
Higher risk of severe hypoglycemia; the mealtime insulin is reduced when pramlintide starts.

**Oral drugs that need fast absorption (pain relievers, antibiotics, oral contraceptives)**
Slower gastric emptying delays them; they are taken well before or after the injection.

## What is monitored

- Capillary glucose before and after meals and at bedtime
- Nausea and food intake

## Administration and safety

- Subcutaneous injection in the abdomen or thigh before major meals.
- Never mixed with insulin in the same syringe (different pH).
- Not given if the meal will be skipped or is very small.
- The solution should be clear and colorless.

## Nursing care

- Before: history of gastroparesis and of severe or unnoticed hypoglycemia.
- During: glucose checks in the hours after injection; watch for nausea.
- Teach: recognize hypoglycemia and avoid driving or machinery until the response is known.

## Pregnancy and breastfeeding

Data in pregnancy are limited; insulin alone is the usual treatment.

## Memory aid

Pramlintide restores insulin's 'quiet partner' amylin: slow stomach, less glucagon, earlier fullness. Partner plus insulin can drop glucose hard, so insulin is cut first.

## Test yourself

Think of the answer, then tap the question to see it.

## Why is the mealtime insulin dose reduced when pramlintide is started?

Pramlintide plus insulin can cause severe hypoglycemia, especially in the first hours after injection.

## Which digestive condition is a contraindication to pramlintide?

Gastroparesis, because pramlintide slows gastric emptying further.

## Can pramlintide and insulin be drawn into the same syringe?

No. Their solutions have different pH and are injected separately.
