# Tedizolid

A once-daily oxazolidinone, oral or IV, for gram-positive skin infections including MRSA; similar to linezolid.

Updated 2026-11-01

- Class: [Oxazolidinones](https://durgical.com/en/pharmacology/classes/oxazolidinones/)
- Target: Peptidyl-transferase centre of the 23S rRNA, 50S ribosomal subunit
- ATC code: J01XX11

◆**Peptidyl-transferase centre of the 23S rRNA, 50S ribosomal subunit**Tedizolid

ⓘ The figure shows tedizolid in the 50S ribosomal subunit of methicillin-resistant Staphylococcus aureus.

**Target in 3D:** Peptidyl-transferase centre of the 23S rRNA, 50S ribosomal subunit. PDB structure 6WRS (RCSB PDB, public domain). [View the structure at RCSB PDB](https://www.rcsb.org/structure/6WRS) · Drag to rotate · pinch to zoom

## Mechanism of action

Tedizolid binds the 23S rRNA of the 50S subunit, like linezolid, and stops bacterial protein synthesis, which halts growth. Its spectrum is similar to linezolid's: staphylococci (including MRSA), streptococci and enterococci.

- Binds the 23S rRNA of the 50S subunit
- Bacterial protein synthesis stops
- Gram-positive growth stops

## Pharmacokinetics

**Absorption**
Tablets or IV infusion, once a day; it can be taken with or without food.

**Metabolism**
Not a substrate or inhibitor of cytochrome P450 enzymes.

## Indications

- Acute bacterial skin and skin-structure infections by gram-positives, including MRSA. [Cellulitis of the leg →](https://durgical.com/en/conditions/cellulitis-of-the-leg/)

## Adverse effects

### Common

- Nausea, vomiting, diarrhoea, headache

### Serious (rare, important)

- Possible effects on blood counts; early data suggest less myelosuppression than linezolid
- Serotonin syndrome with serotonergic drugs
- Clostridioides difficile diarrhoea

## Contraindications

- Hypersensitivity to tedizolid

## Key interactions

**SSRIs and other serotonergic drugs**
Possible serotonin syndrome, though preclinical data suggest a lower risk than with linezolid.

## What is monitored

- Blood count in patients with prior low white cells
- Signs of serotonin syndrome with serotonergic drugs

## Administration and safety

- Oral and IV doses are given once daily at about the same time.
- The full short course is completed even if the skin improves.

## Nursing care

- Review antidepressants and other serotonergic drugs before the first dose.
- Ask about previous low white blood cell counts.
- Teaching: report fever, sweating, agitation, tremor or diarrhoea.

## Pregnancy and breastfeeding

Human data are limited; the pregnancy is reported to the treating team, which weighs benefit and risk.

## Memory aid

TEDIZOLID is «linezolid on a diet»: same ribosome site, one dose a day, perhaps lighter on marrow and serotonin.

## Test yourself

Think of the answer, then tap the question to see it.

## What does tedizolid share with linezolid?

The target (23S rRNA of the 50S subunit) and a similar gram-positive spectrum, including MRSA.

## Which interaction is still watched for with tedizolid?

Serotonin syndrome with serotonergic drugs such as SSRIs.

## Is tedizolid metabolized by cytochrome P450?

No; it is neither a substrate nor an inhibitor of CYP enzymes.
