# Visceral and referred pain

- Mechanism
- Exam priority: medium

Organs hurt in a vague, deep way, and the brain often blames the skin that shares the same spinal segment.

Also called: Referred pain, Convergence–projection

## Key points

- Viscera are insensitive to cutting or burning but very sensitive to distension, ischemia, chemical irritation and spasm.
- Visceral afferents run with sympathetic (splanchnic) nerves to thoracolumbar segments; pelvic organs also use the pelvic splanchnic nerves (S2–S4).
- Referred pain = convergence of visceral and somatic afferents on the same spinal neuron; the brain reads it as skin pain.
- Once the parietal peritoneum or pleura is inflamed, pain becomes sharp and well localized (somatic nerves).
- Diaphragm irritation refers to the shoulder because the phrenic nerve enters at C3–C5.

## What it is

Visceral pain comes from hollow organs and capsules stretched, inflamed or starved of blood. It travels on few, slow C fibres that run with autonomic nerves, so it is dull, poorly localized and comes with nausea and sweating. Because those fibres meet skin fibres on the same dorsal horn neurons, the brain often projects the pain to the skin of that dermatome: referred pain.

## How it works, step by step

- Distension, ischemia or inflammation activates free endings of thin C fibres in the organ wall or capsule.
- These afferents travel with sympathetic nerves through the sympathetic trunk to the dorsal root ganglion of their segment.
- In the dorsal horn they synapse on the same second-order neurons that receive skin afferents from that dermatome.
- The anterolateral (spinothalamic) pathway carries the signal up; the cortex has a fine map of skin but a crude one of viscera.
- The brain assigns the pain to the familiar source, the dermatome, so it is felt at the body surface (referred).
- When inflammation reaches the parietal lining, somatic nerves fire and the pain becomes sharp and point-like.

## Classic referral sites (segment of entry)

| Organ | Where it is felt | Segments |
| --- | --- | --- |
| [Heart (ischemia)](https://durgical.com/en/conditions/myocardial-infarction/) | Retrosternal, left arm, jaw, neck | T1–T4 (and C8) |
| [Appendix (early)](https://durgical.com/en/conditions/acute-appendicitis/) | Periumbilical, then right lower quadrant when peritoneum is reached | T10 |
| [Gallbladder](https://durgical.com/en/conditions/acute-cholecystitis/) | Right upper quadrant, tip of right scapula; right shoulder if diaphragm is irritated | T7–T9; C3–C5 via phrenic |
| Stomach and duodenum | Epigastrium | T6–T9 |
| Kidney and ureter | Flank to groin | T10–L1 |
| Diaphragm (central) | Shoulder tip | C3–C5 |

## Clinical notes

- Myocardial infarction can present as arm or jaw pain alone, especially in people with diabetes and older adults, who may also have silent ischemia.
- Appendicitis: pain migrates from the navel to the right iliac fossa as the parietal peritoneum becomes inflamed.
- Blood under the diaphragm (ruptured spleen, ectopic pregnancy) can cause shoulder-tip pain.

## Memory aid

The spinal cord has one mailbox per segment: a letter from the heart lands in the same box as letters from the arm, so the brain reads the arm's address.

## Test yourself

Think of the answer, then tap the question to see it.

## Why is visceral pain dull and poorly localized while skin pain is sharp and precise?

Visceral afferents are few, slow C fibres spread over several segments, and the cortex has only a crude map of viscera; skin has dense receptors, fast Aδ fibres and a detailed cortical map.

## Why does early appendicitis hurt around the navel and later in the right lower quadrant?

Early pain is visceral, referred to the T10 dermatome (umbilicus). When inflammation reaches the parietal peritoneum, somatic nerves localize it to the right iliac fossa.

## Why can gallbladder disease cause right shoulder pain?

Inflammation that irritates the diaphragm stimulates phrenic afferents, which enter the cord at C3–C5, the same segments that supply the skin of the shoulder.

## Which stimuli activate visceral nociceptors, and which do not?

Distension, ischemia, chemical irritation and smooth muscle spasm activate them; cutting, crushing or burning a viscus often causes little pain.
