Primary vs secondary response and memory !

  • Process

The first time the body meets a germ it responds slowly; the second time, memory cells make a faster and stronger response.

Updated 2026-11-01

What it is

On first exposure (primary response), naive lymphocytes need days to expand, and the first antibodies are mainly IgM. Some activated cells become long-lived memory B and T cells and long-lived plasma cells. On re-exposure (secondary or anamnestic response), memory cells respond within days, producing much more antibody, mostly class-switched IgG with higher affinity. This is the basis of vaccination.

Step by step

  1. First exposure: antigen activates rare naive clones in lymph nodes.
  2. After a lag of several days, antibodies appear, mainly IgM at first.
  3. Class switching to IgG and affinity maturation happen in germinal centers over the following weeks.
  4. Memory B cells, memory T cells and long-lived plasma cells (bone marrow) remain after the infection clears.
  5. Re-exposure: memory cells, more numerous and easier to activate, respond with a shorter lag.
  6. The secondary response gives more IgG, of higher affinity, and lasts longer.

Clinical notes

  • Booster doses of vaccines use the secondary response to raise and prolong protection.
  • Specific IgM points to a recent or current infection, for example IgM against hepatitis B core antigen in acute hepatitis B.
  • A fourfold rise in IgG between acute and convalescent samples supports a recent infection.
  • After measles, the virus can erase part of existing immune memory, which increases later infections.

Think of the answer, then tap the question to see it.

Which antibody class predominates early in the primary response?

IgM.

What are three differences between the secondary and the primary antibody response?

The secondary response has a shorter lag, a larger amount of antibody that is mainly IgG, and higher affinity, and it lasts longer.

Where do many long-lived plasma cells reside?

In the bone marrow, where they keep secreting antibody for years.

What does a positive specific IgM usually indicate in serology?

A recent or current infection, because IgM is made early in the primary response.
