Consolidation syndrome !

  • Syndrome

When the air sacs fill with fluid or cells, the lung becomes solid and sounds and vibrates differently on exam.

What it is

In consolidation, alveolar air is replaced by exudate, blood, cells or tissue while the bronchi stay open. Solid lung carries sound and voice better, so fremitus increases and bronchial breathing appears. The most common cause is bacterial pneumonia.

Key findings

  • Early or central pneumonia may have a normal exam.
  • A chest radiograph confirms consolidation and shows air bronchograms; lung ultrasound also detects it.

Pathophysiology

  • The alveoli fill with inflammatory exudate (pneumonia), blood (infarction, contusion) or tumour cells, and the air is lost.
  • The bronchi of the segment or lobe stay open and air-filled, so they carry sound to the wall.
  • Solid tissue passes the high frequencies unfiltered: bronchial breathing, a clear voice and stronger fremitus.
  • The perfused but unventilated area creates a shunt and hypoxaemia.

Common causes

  • Community-acquired pneumonia (pneumococcus is the classic bacterial cause).
  • Hospital-acquired pneumonia and aspiration pneumonia.
  • Pulmonary tuberculosis with extensive infiltrates.
  • Pulmonary infarction from embolism, lung contusion and lung cancer.

Tests that confirm it

  • Chest radiograph: opacity with air bronchograms.
  • Lung ultrasound: consolidation with bronchograms, useful at the bedside.
  • Blood count, oxygen saturation and, when needed, arterial blood gases to judge severity.

Think of the answer, then tap the question to see it.

Why does fremitus increase in consolidation?

Because solid lung with an open bronchus carries the voice vibration better than aerated lung.

Where is the trachea in a pneumonic consolidation?

Midline; there is no major loss or gain of volume to shift the mediastinum.

What happens to the findings if a tumour blocks the bronchus of the consolidated area?

Fremitus, breath sounds and voice sounds fall, and the exam resembles atelectasis.
